During the onboarding process, we gather lots of information about the client, from them and family members, however, it’s over the coming weeks and months, that the care professional gains so much more detail, which they also feed back to the office team to improve the quality and depth of the care plan.

We have also created a client and family portal for direct feedback.

Care Monitoring for Family and Friends

The openPASS app provides much-needed transparency through a family care monitoring app for family, friends, and pre-authorised interested parties who require access to up-to-date information about those receiving care, thereby closing the loop to all-in-one care planning.

At Living at Home, we take an inclusive view of care and extend the functionality of digital care management to an individual’s care network via openPASS.

The care notes are available, in real time to family, care professionals such as district nurses, first responders, emergency services, GPs, and therapists. This is a unique application that provides verified parties with inclusive access to real-time care plans and records, the industry standard for care management and monitoring.
